Requirements for Guest Dosing

To ensure a seamless experience, please arrange guest dosing in advance and provide:

  • Verification from your home clinic, including:
    • Current medication and dosage information
    • Dates of last and next dose
    • Prescriber authorization for guest dosing
  • Photo identification (driver’s license, passport, or state-issued ID)
  • Payment arrangements (Medicaid, commercial insurance, or private pay accepted)
  • Contact information for your home program, so we can coordinate care directly

All guest dosing services must be approved and scheduled through coordination between Alliance Wellness Clinic and your home OTP.

How to Arrange Guest Dosing

  1. Contact your home clinic to request a guest dosing transfer.
  2. Have your clinic send records directly to Alliance Wellness Clinic.
  3. Confirm your dosing schedule with our admissions team.
  4. Arrive on your scheduled day with proper ID for verification.
